The following policy is designed to help us do so.
Initial visit: Initial assessment and Treatment, 90 minutes. $135.00
Follow-up visit: Follow-up assessment and Treatment, 60 minutes. $95.00
Maintenance visit: Acupuncture only, 30 minutes. $50.00
MAINTENANCE VISIT: maintenance visits must be pre-approved by clinic staff. To qualify for a maintenance visit you must have already had an initial office visit. The visit does not include consultation and is usually conducted seated and in a shared room.
BILLING: payments are due at time of service. Pharmacy items such as herbs and supplements must be paid for upon receipt. Payment can be in the form of check, cash, visa, or MasterCard, AMEX, or Discover.
INSURANCE: If your treatment is covered by health insurance, you may use your receipt to bill your insurance company by attaching the receipt to your claim form and mailing both to your insurance company. All information needed by insurance companies for reimbursement has been included on the receipt.
CANCELLATIONS: If unable to keep an appointment, please give us 24 hours notice. If you fail to keep your appointment or cancel without at least 24 hours notice, you may be charged a full office fee.
